How It Works

Call the MVD to Check Eligibility

Pre-Screening Appointment

30-minute session to determine eligibility ($50 fee).
Upload your Extended Certified Motor Vehicle Record (MVR) and Photo ID.
Provide treatment certificates and notarized letters of sobriety.

60-minute appointment ($250 fee).
