Create an Account
To conveniently access online forms, make payments, and review your attendance records and receipts, follow the steps below to create your Patient Portal:
In order to get started, please contact ABC at (602) 788-1116 and provide them with your email address. A representative will then send you an email from 'Epitomax.net' with a link that guides you to "follow this link to create a PATIENT PORTAL ACCOUNT". Once your account is created, you will have the convenience of filling out screening and intake forms online, as well as the ability to make payments and review your attendance and payment receipts.
Referrals
There are two options available to request referral forms. You can directly call our administrative office at 602-788-1116 to request customized referral forms for your Court, which can include the Court name, address, and phone number on either 2-print or 3-print carbon copy forms. Alternatively, you can click the link below to download and print a single form, which can be faxed or emailed back to our office.
Out of State Referrals
Easily access our out-of-state referrals and review court documents, ensuring that you have all the resources you need at your fingertips. Our commitment to exceptional customer service extends to our approach of accepting most clients, as we strive to provide accessible services to a diverse range of individuals.
To get started, please download and print our simple Referral Form and Release of Information forms. Once you have completed the forms, you have two options for submission. You can either fax them to 602-788-1119 or scan and send them via email to mrube@azbcinc.com with the subject line: Referral Form.
